Hearing Assessment
Our exclusive Personalized Hearing Health Assessment (PHHA) puts you in control of deciding the best solution for your individual hearing needs. By completing this simple assessment, you provide your Beltone hearing care professional with important information needed to make the right recommendations for your lifestyle.
To open the PHHA form, click here or on the image to the right. Once opened, select your answers to the questions. When finished, simply print out the completed form and take it to your local Beltone Hearing Care Center.
Your Beltone hearing care professional will review the results of your PHHA assessment with you and will give you a hearing screening to find out whether or not you have a hearing loss. He or she will then consult with you on next steps for your hearing care needs.

Hearing Conservation: Preventing Hearing Loss
Extremely loud noises can cause permanent damage to the tiny hair cells inside the cochlea. Even moderately loud noise over a period of time can be damaging. But just how loud is loud? Studies show that prolonged exposure to sounds at or above 90dB can damage hearing. The chart below graphically illustrates some common noises and their respective dB levels.
